SINGLE PLAN
Annual Cost: $790
What You Get:
Diagnostic & X-Rays
- Comprehensive Exam (new patient, initial exam)
- Periodic Exam – 2 per year
- Limited Oral Exam (emergency/problem focused) -1 per year
- Panoramic X-Ray (as needed) – up to 1 per year
- X-Rays (bitewings & periapical) – 1 set per year
Preventative Care
- Adult Teeth Cleaning (Prophylaxis) – 2 per year
- Fluoride Treatments – 2 per year
- Full Mouth Debridement – 15% savings
- Additional Adult Cleaning – 15% savings
Other Procedures
All other dental procedures and treatment needs are discounted 15% up to $600
Coverage: 100% of your essential diagnostic, x-rays, & preventative services are covered, PLUS additional 15% savings on other treatments.

12 & UNDER PLAN

- Perio Maintenance Cleaning – 4 per year
- Flouride Treatment – 4 per year
- Periodic Exam – 2 per year
- X-rays (bitewings & periapical films) – 1 per year
- Panoramic X-ray (as needed) – up to 1 per year
Coverage: 100% of periodontal and preventative services, PLUS additional 15% savings up to $600 on other procedures and treatments if needed.

EXCLUSIONS AND LIMITATIONS
This program is a discount plan, not a dental insurance plan. It cannot be used:
PLAN TERMS
- Non-Refundable: No refunds or premium will be issued at any time if the plan is unused.
- 12-Month Coverage: All covered services must be used within 12 months of the plan start date.
- Payment Due: Patient’s portion of treatment costs is due on the day of service.
- Appointment Policy: 48-hour notice required for change to scheduled appointments.
